Actually during detail engineering there things get clarified, normally as said above before freezing size of trafo, starting voltage drop at the bus is taken in consideration.

and normally temp. rise (of winding) of trafo is limited as per IS/IEC standards, these are normally specified in technical specification at ordering stage.

Let me tell with an example,
For an ambient temperature of 50 Deg C
Temp. Rise at full load on nominal tap is restricted to:
winding - 45 Deg C
Oil - 35 Deg C
